29 CFR 1910.134(f)(2) as referenced by 29 CFR 1926.103: Employee(s) using tight-fitting facepiece respirators were not fit tested whenever a different respirator facepiece (size, style, model or make) was used:   a.)  VersiTech, Inc., Eagle Natrium Powerhouse:  On or about 03/15/16, employees wearing the 3M 6100 half mask respirator had not been fit tested to wear the negative pressure respirator.

29 CFR 1926.62(d)(2)(iii): With respect to the tasks listed in 29 CFR 1926.62(d)(2)(iii)(A) and (B), where lead was present, until the employer performed an exposure assessment as required in 29 CFR 1926.62(d), and documented that the employee performing the listed tasks was not exposed in excess of 500 micrograms per meter cubed, the employer did not treat the employee as if the employee was exposed to lead in excess of 500 micrograms per meter cubed and did not implement employee protective measures as prescribed in 29 CFR 1926.62(d)(2)(v):  a.)  VersiTech, Inc., Eagle Natrium Powerhouse:  On or about 03/15/16, employees performing needle gun removal of lead containing paint without the use of a dust collection system were not provided with full face respirators for interim protection.
