29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a-Pump Jack Scaffold:  An employee was observed stepping from a pump jack scaffold to an unprotected roof in which a means of fall protection was not utilized, exposing the worker to fall hazards of approximately 16ft to the concrete below. Condition noted on or about 03/10/2015.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing access.  a- Pump Jack Scaffold: Employees were exposed to fall hazards of approximately 16ft to the concrete below as a ladder used for access, whose side rails did not extend 3ft above the upper landing surface. Condition noted on or about 03/10/2016.
