29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury.  On or about 16 March 2016, and at times prior thereto, 2 employees were working in a trench that measured approximately 222 feet long by 4 feet wide and 7 feet deep without the employer providing training on the hazards associated with excavations, exposing the employees to hazards such as, but not limited to cave-in and struck-by.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees.  On or about 15 March 2016 and times prior thereto, 2 employees were performing utility installation activities in an excavation, which measured approximately 222 feet long by 4 feet wide by 7 feet deep, without a safe means of egress, exposing the employees to the hazard of being caught in a cave-in.

On or about 15 March 2016 and times prior thereto, 2 employees were performing utility installation activities in an excavation, which measured approximately 222 feet long by 4 feet wide by 7 feet deep, that had not been inspected by a competent person, exposing the workers to the hazard of being caught in a cave-in.
