29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. On or about March 17, 2016, during the installation of masonry material the employer did not ensure employees working from a Waco fabricated frame type scaffold were protected from falling to a lower level by a guardrail, personal fall arrest, or other fall protection system. The employees were working approximately 15 feet above the ground. JIM KOCAN MASONRY, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.451(g)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1052742, CITATION NUMBER 01, ITEM NUMBER 001, AND WAS AFFIRMED AS A FINAL ORDER ON MAY 7, 2015, WITH RESPECT TO A WORK PLACE LOCATED AT WEST 17TH STREET AND BRADFORD, CLEVELAND, OHIO.
