29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    The employer did not establish an energy control program including written energy control procedures, employee training, and periodic inspections to ensure employees performing servicing and maintenance on machines and equipment including the custom fabricated shearing machines in the doctor blade room and the OMAX 2626 JetMachining Center in the end seal room were protected from unexpected energizing, startup, or release of energy.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    Four custom fabricated shearing machines located in the doctor blade room were not provided any form of guarding to prevent the operator and other employees from hazards created by the point of the operation of the shear, ingoing nip points on the in-feed rollers, and ingoing nip points created by the drive belt and pulleys for the rollers and re-winder.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    (a) Shearing machine one located in the doctor blade room did not utilize a relocatable power tap in accordance with Underwrite Laboratories Inc. listing, UL 1363 Relocatable Power Taps, as it was affixed to the workbench with metal clips and powered industrial equipment.     (b) Shearing machine two located in the doctor blade room utilized two metallic outlet boxes (handy boxes) mounted with brackets onto the workbench.  The handy boxes which were not installed in accordance with the UL identifier QCIT which states the boxes shall be mounted in or on a surface as the boxes were equipped with brackets that are designed to be installed on wood or metal studs in new construction.
