29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section: On or about 10 March 2016, the employer's lockout/tagout program did not include documented procedures that clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of potentially hazardous energy. Employees gaining access to the equipment, including bagger machines, to perform servicing and maintenance were exposed to caught-in hazards. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)

29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. On or about 10 March 2016, the employer failed to effectively train employees to identify each potential energy source and isolate those sources during servicing and cleaning operations on equipment, including bagger machines. 29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): On or about 10 March 2016, the employer did not ensure that energy isolating devices were not operated to control/isolate potentially hazardous energy while an employee was cleaning a bagger machine in the Ingredient Division. Uncontrolled energy sources, including pneumatic air pressure, thereby exposed employees to amputation hazards. Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).
