29 CFR 1926.501(b)(3): Each employee in a hoist area shall be protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems or personal fall arrest systems. If guardrail systems, [or chain, gate, or guardrail] or portions thereof, are removed to facilitate the hoisting operation (e.g., during landing of materials), and an employee must lean through the access opening or out over the edge of the access opening (to receive or guide equipment and materials, for example), that employee shall be protected from fall hazards by a personal fall arrest system.    (a) On March 17, 2016, employees performing roofing work on a flat roof were exposed to a fall hazard of approximately 22.5 feet without the benefit of fall protection.    The Cairo & Sons Roofing Co., Inc. dba CSR Roofing Contractors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 92249, citation number 1, item number 2 and was affirmed as a final order on August 17, 2011, with respect to a workplace located at 735-773 Donna Court, Wheeling, IL  60090/    In accordance with 29 CCFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
