29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent):  a) Plant, Airlay - On or about 03/08/16 and at times prior thereto employees climbed on the top of the Bale Breakers to clean the machines with compressed air. No fall protection equipment was in use to prevent falls from a 10 ft. elevation onto the concrete floor.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  a) Airlay - On or about 03/08/16 and at times prior thereto employees were assigned to clean equipment with compressed air under group lockout/tagout performed by a team leader. Employees were not trained in the recognition of the type and magnitude of the hazardous energies for each specific equipment, and the methods and means necessary for energy isolation. Workers were exposed to electrical energy, steam, compressed air, hot surfaces and fluids, crushes and amputations.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, and rotating parts:  a) Plant, BMW Foam Clamp - On or about 03/08/16 and at times prior thereto employees  were instructed to place their bodies inside presses #1, 2, and 3 to clean the molds between cycles. The light curtains failed to detect the presence of the operators inside the presses.     b) Plant, Airlay - On or about 03/08/16 and at times prior thereto the side of the Laroche tearing machine had an incomplete guard, exposing workers to nip points created by a rotating drum and a spiked roller.    c) Plant, Light Weight Foam Molding - On or about 03/08/16 and at times prior thereto the Oven Press #5 was not guarded to protect employees and other workers from accidental contact with the cycling hot press.   d) Plant, Light Weight Foam Molding - On or about 03/08/16 and at times prior thereto the Oven Press #6 was not guarded to protect employees and other workers from accidental contact with the cycling hot press.   e) Plant, Light Weight Foam Molding - On or about 03/08/16 and at times prior thereto the Oven Press #7 was not guarded to protect employees and other workers from accidental contact with the cycling hot press.   f) Plant, Airlay - On or about 03/08/16 and at times prior thereto the side of the machine was not guarded to prevent contact with the horizontal auger.  g) Plant, Airlay - On or about 03/08/16 and at times prior thereto the side of the machine was not guarded to prevent contact with the brake rotor.   h) Plant, Light Weight Foam Molding - On or about 03/08/16 and at times prior thereto the light curtains in the back of press #6 failed to detect the presence of the back operator during cycles initiated by the front operator.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.303(b)(7)(iv): There were damaged parts that adversely affected safe operation or mechanical strength of the equipment, such as parts that were broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating:  a) Plant, Light Weight Foam Molding - On or about 03/08/16 and at times prior thereto the nominal 208 V dead front panel mounted on a structural steel column (in front of Oven Press #7) was crushed by a forklift and was left connected in service.   b) Plant, Airlay Building - On or about 03/08/16 and at times prior thereto the conduit protecting the power cable in the Laroche machine  was broken and separated in proximity of the strain relief device (480 V motor). Employees were exposed to potential loss of insulation and contact with 480 live parts.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
