29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. a) Mining Control: On or about March 17 and times prior, an employee performed energy control procedures on machines such as but not limited to Accurshear Shear and Strippit Punch Press without having a periodic inspection of the energy control procedures.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. a) Fabrication Area: On or about March 17, 2016 the employer's energy control procedures did not contain specific procedural steps for the Accurshear Shear to control hazardous energy sources such as 3000psi hydraulic pressure. b) Fabrication Area: On or about March 17, 2016 the employer's energy control procedures did not contain specific procedural steps for the Strippit Punch press to control hazardous energy sources such as 120psi air.
