The attachment point of the body narness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  a) On or about March 15, 2016; roof of building; employees were installing shingles on a 10:12 pitch commercial roof without properly attaching the lanyard to the "D" ring located in the center of the back; exposing them to a 40 foot fall .   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
