29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a. On or about 03/07/2016, at the job site - employees replacing ceiling light fixtures were exposed to an electric contact hazard, in that, the electric source was not de-energized and lockout and taggout devices were not attached. One employee was injured after making contact with the energized wire which caused him to fall from a ladder.
