29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Note:Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.  a) Pipe area: The employer did not provide guardrails around the upper level storage area of the Pipe Room, exposing employees to a fall hazard of approximately 7.5 feet.

29 CFR 1910.36(d)(1): Employees must be able to open an exit route door from the inside at all times without keys, tools, or special knowledge. A device such as a panic bar that locks only from the outside is permitted on exit discharge doors.  a) Pipe room:  Employees packing boxes were not able to the exit the facility in the event of an emergency due to an emergency exit door that required more than one action and was not able to be opened easily.  b) Main warehouse: Employees filling orders were not able to exit the facility in the event of an emergency due to an emergency exit door that required more than one action and was not able to be opened easily.
