29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) Leisure Oaks Subdivision, 47 Oak Ln, Douglasville, GA. A work crew of five employees was removing roof shingles from a 7/12 pitch roof were exposed to fall hazards of 22 feet 3 inches to the back patio of the residence.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) Leisure Oaks Subdivision, 47 Oak Ln, Douglasville, GA at the front porch. A work crew of two employees was removing roof shingles, utilizing an extension ladder that extended approximately 18 inches above the upper landing, exposing employees falls of 15 feet 9 inches to the ground.    (b) Leisure Oaks Subdivision, 47 Oak Ln, Douglasville, GA. at the rear patio. A work crew of three employees was removing roof shingles, utilizing an extension ladder that extended approximately 2 feet above the upper landing, exposing employees to falls of 22 feet 3 inches to patio below.
