29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:      (a)   On or about February 22nd, 2016 an employee of A & B Improvements sustained an injury after falling off an eight foot high roof with a 5/12 pitch while shoveling snow without the benefit of fall protection in the vicinity of 2430 Tenth Avenue SW in Jamestown, North Dakota.      (b)   On or about February 22nd, 2016 and at times prior, employees of A & B Improvements were exposed to up to a 12 foot fall hazard on a 5/12 pitch roof while shingling along edges of dormers and gables without the benefit of fall protection in the vicinity of 2430 Tenth Avenue SW in Jamestown, North Dakota.

29 CFR 1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required:  (a)   On or about February 22nd, 2016 and at times prior, employees of A & B Improvements were exposed to up to a 12 foot fall hazard on a 5/12 pitch roof while shingling along edges of dormers and gables without the benefit of fall protection in the vicinity of 2430 Tenth Avenue SW in Jamestown, North Dakota.
