OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, exposing employees to amputation hazards. a). On or about March 16, 2016 employees operating the NRM 4-1/2" Cold fed rubber extrusion machines are exposed to amputation hazards because the process the employee uses to attach the hose end of one reel to the beginning hose of the next reel for continuous feed into the machine, created a condition where an employee's thumb was entangled in the thin Kevlar cord used to tie off the two ends, resulting in amputations of the thumb.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a). On or about March 16, 2016 employees are conducting reel to reel changes for continuous extruded rubber application to high pressure rubber hose. While the machine is still running the employee must use a cord to tie the end of one hose reel to the beginning of the next hose reel ensuring continuous operation of the extruder machines without shutting the machines down.
