On or about March 12, 2016, the TGM S400 tube filling machine on line 9 was not locked  out while being serviced, exposing employees to an amputation hazard.

29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources:  a.  On or about March 12, 2016, at the TGM S400 tube filling machine on line number 9, an employee working as an authorized employee was not provided the equipment necessary to guard against all sources of energy, exposing the employee to an amputation hazard.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:    a.     On or about March 12, 2016, at the TGM S400 tube filling machine on line 9,  an employee trained as an affected employee was working as an authorized employee, but was not provided authorized employee training, exposing the employee to an amputation hazard.
