29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: a) East Columbus Ave. (between Bliss & Howard Sts.), Springfield, MA: On or about March 11, 2016, an employee was not protected from tripping in or stepping into an open manhole adjacent to the working/walking surface he was standing on/working from to perform his work activity where he had set up his laser level on a tripod. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system: a) East Columbus Ave. (between Bliss & Howard Sts.), Springfield, MA: On or about March 11, 2016, an employee was working in an unprotected section of the excavation at this location that was in excess of 5 feet deep. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1). Northern Construction Services, LLC was previously cited for a repeat violation of this occupational safety and health standard 29 CFR 1926.652 (a) 1, which was contained in OSHA inspection number 895960, citation number 2, item number 1 and was affirmed as a final order on July 9, 2013, with respect to a workplace located at 158 Hicks Way (UMASS), Amherst , MA.
