29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about March 15, 2016, on the south side of the restaurant where employees were exposed to a fall hazard greater than 6 feet when installing the roofing material without protecting a hatch floor opening with a cover, guardrail system or personal fall arrest system.
