29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident On or about March 8, 2016, an employee working on an extension ladder was hospitalized after falling from a height of approximately 15 feet with the injuries becoming fatal on March 11, 2016. The employer reported the fatality on March 15, 2016 and therefore failed to report the hospitalization within the required 24 hours and the death within the required 8 hours.
