29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph.  a) Jobsite - Employees were exposed to tip over hazards while conducting masonry operations from a supported tubular welded frame scaffold measuring approximately 23 feet 7 inches in elevation, and 5 feet 1 inch in width, at a height approximately 20 feet above ground level, as it was not restrained from tipping or falling over by guying, tying, bracing, or equivalent means.   Observed on or about 3/10/16.

29 CFR 1926.452(c)(2): Frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which secure vertical members together laterally, and automatically square and align the vertical members so that the erected scaffold is always plumb, level, and square.  a) Jobsite - Employees were conducting masonry operations from a supported tubular welded frame scaffold, approximately 20 feet above ground level, without secured, squared, and aligned vertical members as it was not braced by cross, horizontal, or diagonal braces, or combination thereof.  Observed on or about 3/10/16.

29 CFR 1926.451(f)(4): Part(s) of a scaffold that were damaged or weakened, such that the scaffold's strength was less than that required by paragraph (a) of this section, were not immediately repaired, replaced, braced to meet those provisions, nor removed from service until repaired.  a) Jobsite: End frame over staircase - The damaged and weakened end frame leg of the supported tubular welded frame scaffold, with approximately 30 inches of its original frame cut out,  was not immediately repaired or replaced, or braced to support its own weight or 4 times the maximum intended load, or removed from service until repaired.   Observed on or about 3/10/16.

The employees were conducting masonry operations 20 feet above ground level from a scaffold missing approximately 30 inches from one leg of one of the end frames.  Observed on or about 3/10/16.
