29 CFR 1926.20(b)(1): The employer did not initiate and maintain an accident prevention program as may be necessary to comply with this part.    a) Jobsite - The employer did not develop and implement an accident prevention program to assist their employees identify and prevent hazards associated with their work operations.  An employee suffered severe facial and arm injuries after putting his arm inside a disengaged mixer that unexpectedly started.  The rotating mixer paddles caught the employees sleeve and pulled him in.    Noted on or about 3/14/16.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and maintain at each workplace, a written hazard communication program which described how the criteria specified in 29 CFR 1910.1200 paragraphs (f), (g), and (h) for labels and other forms of warning, safety data sheets, and employee information and training would be met.    a) Jobsite: The employer did not have a written hazard communication program at the workplace.  Employees were performing masonry operations while handling hazardous chemicals, such as, but not limited to, Portland and masonry cement, silicates, gypsum, and others contained in Keystone Hi-Strength Masonry Cement For Type S Mortar.
