29 CFR 1910.22(d)(2): It shall be unlawful to place, or cause, or permit to be placed, on any floor or roof of a building or other structure a load greater than that for which such floor or roof is approved by the building official. a) On March 7, 2016 while standing on an elevated platform to remove and replace a fan motor in Kiln #9, employees were exposed to fall hazards in that during the movement of a 440 pound motor, the flooring gave way with the employees, exposing them to falls in excess of 20 feet tall.

29 CFR 1910.132(a): Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact. a) On March 13, 2016 fall protection was not used and/or provided for employees who were performing maintenance on Kiln #9, in that employees were exposed to fall hazards in excess of 25 feet.
