29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  (a) 3405 North Front Street, Harrisburg Pennsylvania - An employee worked on a roof bracket scaffolding removing slate from a roof approximately 20 feet above the ground.  The employer did not ensure that the employee was provided with fall protection, on or about March 17, 2015.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

The employer did not ensure that employees were using fall protection while on the roof, when exposed to a 20 foot fall hazard, on or about March 17, 2016.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

The employer did not ensure that the ladder's side rails extended 3 feet above the roof, on or about March 17, 2016.   Abatement certification required within 10 days after abatement date.
