29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a) On or about March 21, 2016; fall protection from scissor lift platform: The employer did not ensure employees working from electric self-propelled scissors lift platforms were protected from fall of approximately 16 feet to the concrete floor due to the mid-rail chain not being attached before elevating the platform. b) On or about March 21, 2016; climbing out from scissors lift basket: The employer did not ensure an employee stayed in the basket of the scissors lift when he climbed out onto the roof support I-beams which exposed him to falls of 18 feet 10 inches to the concrete floor.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: a) On or about March 21, 2016; fall protection: The employer failed to ensure employees conducting steel erection were protected from falls of 18 feet 10 inches while on the roof support I-beam tightening bolts without fall protection.
