29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:       On or about March 21, 2016, employees were exposed to a fall hazard, while accessing and exiting the work area via an extension ladder that did not extend at least 3 feet above the upper landing surface.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.       On or about March 21, 2016, Employees performing roofing activities were exposed to fall a hazard on a construction site in Springfield, Illinois and the employer did not ensure adequate fall protection was being used outside the warning line system.         Meyer Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.20(b)(2), which was contained in OSHA inspection number 1056297 citation number 1, item number 3A and was affirmed as a final order on May 19, 2015, with respect to a workplace located at 424  434 Holton Dr. Bloomington, IL 61701.
