29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) Turnpike Interchanges and Regional Offices throughout the Entire Pennsylvania Turnpike System - Employees worked as armed security officers providing security for the Pennsylvania Turnpike Commission fare collection operation, which involved the delivery and pick up of money. The employer did not require and ensure that these armed security officers wore ballistic-resistant body armor vests, exposing employees to ballistic injuries to the vital organs in the torso, on or about March 20, 2016. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1904.39(a): The employer failed to report to OSHA a work-related death within eight (8) hours after the death of the employee: (a) Pennsylvania Turnpike Interchange 180 - Fort Littleton, Fort Littleton, Pennsylvania - An employee died on March 20, 2016 as a result of multiple gunshot wounds to the torso on the referenced job site on March 20, 2016, and the employer failed to report to OSHA this work-related death within eight hours after the death. (The employer reported the death to OSHA on March 22, 2016.) Corrected during the inspection.
