29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    a) On or about March 22, 2016 CHSO observed two employees on the rooftop cutting and securing plywood sheathing to the roof trusses, using pneumatic nailers and portable electric saws and were not protected from physical impact injuries to the eyes because they were not wearing any eye protection.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    a) On or about March 22, 2016 four employees were exposed to electrical shock hazards while operating portable electric 7-1/4" power saws because the flexible extension cords they were using to energize the saws did not have a continuous path to ground, because the ground pin contact had been broken off or removed.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a)  On or about March  22, 2016, two employees were nailing new roof sheathing to roof trusses and were not protected from falls in excess of 12 feet to the concrete slab below because they were not protected by any means of personal fall protection harnesses, guardrails, or safety nets.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) On March 22, 2016 two employees were working on the roof top and were not protected from falling off the roof to the ground below while getting on or off of the ladder because the ladder was not properly positioned to allow for safe access to the roof surface.

29 CFR 1904.40(a): The employer did not provide an authorized government representative the records within the four business hours.    a). On March 22, 2016 CSHO requested copies of the companies OSHA 300 and 301 logs for review.  On March 25, 2016 during a telephonic closing conference with the employer, CSHO again requested the OSHA logs be emailed or faxed to CSHO for review. On or about March 30, 2016, CSHO received the companies OSHA 300 logs only.

CSHO observed 2 employees working on a steep slope 6:12 pitch, residential roof without any means of personal fall protection, guard rails or safety nets.
