29 CFR 1926.405(a)(2)(ii)(E): Lamps used in temporary wiring for general illumination were not protected from accidental contact or breakage:    a) On or about March 16, 2016, employees traversing the stairwell to gain access to the 5th floor were exposed to electrical shock hazards because the temporary lights in the stairwell affixed to the handrails had missing cages resulting in broken and missing light bulbs that exposed the metal contacts of the socket.

29 CFR 1926.405(g)(2)(iii): The repair splice in a hard service flexible cord No. 12 or larger did not retain the insulation, outer sheath properties, and usage characteristics of the cord which was being used:    a)  On or about March 16, 2016 employees using power tools to set concrete form tables on the 5th floor were exposed to electrical shock hazards because the temporary flexible power cord in use was cut and spliced at the section before the adapter plugged into the outlet box.  This splice exposed the wires by removing the insulation of the cord.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a). On or about March 16, 2016 employees working on the project were exposed to falls in excess of 45 feet to the concrete below because the location of their jobbox tool box maintained on site was within 3 feet of the floor edge and was not protected by means of a guard rail or safety net.   b). On or about March 16, 2016 employees using a Porta Potty temporary bathroom were exposed to fall hazards in excess of 35 feet to the ground below because the Porta Potty was positioned within 2 feet of the floor edge where no guardrails or safety nets were in place.

29 CFR 1926.502(b)(4): When the 200 pound (890 N) test load specified in paragraph (b)(3) of this section was applied in a downward direction, the top edge of the guardrail deflected to a height less than 39 inches (1.0 m) above the walking/working level.    a). On or about March 16, 2016 employees traversing the upper floors of a 5 story construction project were exposed to falls in excess of 45 feet to the concrete below because the cable guardrail system installed at the floor edges were loose and did not maintain a deflected height of 39 inches above the walking level.
