29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  On or about March 15, 2016 five employees were removing existing roof materials on a two story, 7:12 pitch, steep slope roof on a multi-unit residential structure, and were exposed to falling over 20 feet to the ground below, because they were not protected by any means of personal fall protection, safety nets, or guard rails.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a)   On or about March 15, 2016 employees were using a portable ladder to access the roof of a two story residential dwelling and were exposed to fall hazards in excess of 8 feet to the ground below, because the side rails of the ladder did not extend at least three feet above the roof line.
