29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. On or about March 21, 2016 and at times prior thereto, four employees were observed working on a residential gable roof without any type of fall protection. This condition exposed employees to falls hazards greater than 6 feet to the ground.

29 CFR 1926.1053(b)(1): The side rails of portable ladders used for access did not extend at least 3 feet above the upper landing surface, or was secured at its top to a rigid support that will not deflect and along with a grasping devise to assist employees mounting and dismounting the ladder. On or about March 21, 2016 and at times prior thereto, four employees were observed working on a residential roof using an aluminum ladder to access the roof. The ladder did not extend at least three feet above the landing surface. This condition exposed employees to fall hazards.
