29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. Construction Site: On or about March 22, 2016 and at times prior thereto, and employee was observed using a flexible cord where the strain relief device (outer sheath) was broken. This condition exposed employees to electrical shock hazards.

29 CFR 1926.1200(e)(1): Employer had not developed or implemented a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met and which also includes the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii): (Construction Reference: 1926.59)(a). Construction Site: On or about March 22, 2016 and at times prior thereto, the employer did not develop or implement a written hazard communication program. Safety data sheets, employee information and training were not provided. This condition exposed employees to potential health hazards.
