29 CFR 1910.305(j)(2)(i): All 15- and 20-ampere attachment plugs and connectors shall be constructed so that there are no exposed current-carrying parts except the prongs, blades, or pins. The cover for wire terminations shall be a part that is essential for the operation of an attachment plug or connector (dead-front construction). Attachment plugs shall be installed so that their prongs, blades, or pins are not energized unless inserted into an energized receptacle. No receptacles may be installed so as to require an energized attachment plug as its source of supply. a. On or about March 22, 2016, located in the office, there was a 3M Wireless Communications System battery charger that had two wires attached directly into an outlet in place of an attachment plug. The wiring exposed the employees an electrical shock hazard of 120 VAC.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets. a. On or about March 22, 2016, located throughout the facility, employees used chemicals for cleaning, i.e. Oven, Freezer, and Restroom Cleaner without having the required training concerning the hazards of the chemicals prior to their use.

On or about March 22, 2016, located in the kitchen, the employer did not ensure that the flexible cord to a 120 volt Wells Manufacturing Company gravy warmer, model number 10MW3TL, had strain relief provided at its plug. b. On or about March 22, 2016, located in the kitchen, the employer did not ensure that the flexible cord of a 120 to a Ram Center, Inc. fry hopper, model number AFD-280E, serial number AFER0205C00338, had strain relief provided at its plug.
