If a gangway is not practical, a substantial straight ladder, extending at least 36 inches above the upper landing surface and adequately secured against shifting or slipping was not provided. When conditions are such that neither of gangway nor a straight ladder can be used, a Jacob's ladder meeting the requirements of (d)(1) and (2) were not used.     (a) On or about March 21, 2016, onboard the Endeavor in the Channel Islands Harbor, Oxnard, California, the employer did not provide a gangway for safe boarding of the vessel. Employees were exposed to a fall hazard.
