29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a)      Intersection of University and Old Shell Road, Mobile, AL:  Employer failed to ensure employees set up portable ladder to extend over 3 feet from upper landing surface to prevent falls of  12 feet to a dirt surface while performing roof decking  job tasks on a roof with a pitch of 6/12.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a)Intersection of University and Old Shell Road, Mobile, AL commercial construction site:  Employer failed to ensure employees used guardrails, safety net or fall arrest system  to prevent falls of 15 feet to a clay surface while performing roofing job tasks on a steep roof with a 6/12 pitch.  L & J Cleaning & Construction Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (B)(13), which was contained in OSHA inspection number 1076024, citation number 1 , item number 1 and was affirmed as a final order on Sept. 8th, 2015, with respect to a workplace located at 56 Pleasant St., Lots 209, 210, 211, Santa Rosa Beach, FL 32459.
