29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) Commercial building, Front side: On or about March 23, 2016, and at times prior, the employer exposed employees to fall hazards of 24-feet, 7-inches from the roof, in that the ladder provided to access the ground level was not extended past the roof at least 3-feet to allow employees safe access to the roof and lower ground level.
