29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:     On or about March 23, 2016, at the above addressed jobsite, the employer failed to maintain, implement, and develop a written hazard communication program. Employees were exposed to eye, skin contact and respiratory tract chemical hazards associated with chemicals, such as but not limited to silica dust.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:   On or about March 23, 2016, at the above addressed jobsite, employees performing masonry demolition work were not trained on the hazards of masonry demolition work, thereby exposing the employees to the hazards associated with silica dust.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:    On or about March 23, 2016, at the above addressed jobsite, employees performing masonry demolition work from a scaffold and were not provided with a safe means of access to the scaffold, exposing employees to fall hazards of approximately twelve (12) to fourteen (14) feet.
