29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):         a) On or about March 23, 2016, employees were exposed to the hazard of falling approximately 21 feet while performing carpentry work from pitched roof deck surfaces without fall protection.  b) On or about March 23, 2016, employees were exposed to the hazard of falling from a second story window opening where fall protection was not provided.         In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.502(d)(6)(i): Snap hooks were engaged directly to webbing, rope or wire rope when not designed for such connections:         a) On or about March 23, 2016, snaphooks used as part of personal fall arrest systems were engaged directly to lifeline ropes when not designed for such connections.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):     29 CFR 1926.602(d): Note: The requirements applicable to construction work under this paragraph are identical to those set forth at paragraph 1910.178(l) of this chapter.       a) On or about March 23, 2016, the employer provided employees with a JCB 507-42 rough terrain forklift without ensuring they were competent to operate the equipment safely, as demonstrated by successful completion of forklift training and evaluation.     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
