29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.  a)  At the jobsite, on the scaffold, on or about 3/22/16:  The employer did not ensure that the scaffold was constructed with fall protection or fall that protection was provided for employees working at 12 feet, 17 feet, and 22 feet from ground.  The employer did not ensure that the scaffold was fully planked at 6 feet, 12, feet, 17 feet, and 22 feet from ground.  The employer did not ensure that the scaffold was built with mud sills when sitting on a soil foundation, and the employer did not ensure that employees had safe access to the scaffold.    Abatement documentation must be submitted for this item.
