29 CFR 1926.304(h)(1): Each circular crosscut table saw shall be guarded by a hood which shall meet all the requirements of paragraph (i)(1) of this section for hoods for circular ripsaws. Location - 7 Athens Street, Cambridge, MA. On or about March 23, 2016 a supervisor and an employee were not protected from injuries when working with a table saw with no guard.

29 CFR 1926.1060(a): The employer shall provide a training program for each employee using ladders and stairways, as necessary. The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards. Location - 7 Athens Street, Cambridge, MA. On or about March 23, 2016 the employer did not provide adequate training to employees who use ladders.
