29 CFR 1926.451(c)(2)(i): Footings were not level, sound, rigid and capable of supporting the loaded scaffold without settling or displacement: (a) Eastside and Southside of Building - On or about March 22, 2016 employees working from welded tubular framed scaffold were exposed to a fall hazard. Scaffold footings were not level, sound, rigid and capable of supporting the loaded scaffold without settling or displacement. Scaffold footing rested on cinder blocks, wood blocks and sections of wood boards.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. (a) Westside and Southside of Building - On or about March 22, 2016 employees working from welded tubular scaffolding were exposed to a fall hazard when attempting to access scaffold. Employees gained access by the use of the framing supports. Ladder was not provided for access.
