29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:   (a) Summit Sealants and Restoration Services Inc. at 1391 Speer Blvd, Denver, CO: On and before March 14, 2016, the employer did not ensure that any employee using a respirator voluntarily was provided with the information contained in Appendix D to this section when the employer determined that any voluntary respirator use was permissible.  The employer provided 3M Model 8511 N95 filtering face piece particulate respirators for voluntary use but did not provide the information contained in Appendix D.  This condition potentially exposed the employees to a respiratory hazard.  Note:  The requirements applicable to construction work under this section are identical to those set forth at 29 CFR 1926.103.

29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form:  (a) Summit Sealants and Restoration Services Inc. at 1391 Speer Blvd, Denver, CO: On and before March 14, 2016, the employer did not identify and evaluate the respiratory hazard in the workplace associated with two employees surfacing concrete on the top of a parking garage with a Blastrac Shotblaster (model 1-8DEZMKII), Bosch Hand Grinder (model 1772-6), and Makita Hand Grinder (model GA7021).  This condition potentially exposed employees to respiratory hazards associated with silica and particulates.  (b) Summit Sealants and Restoration Services Inc. at 1391 Speer Blvd, Denver, CO: On and before March 29, 2016, the employer did not identify and evaluate the respiratory hazard in the workplace associated with two employees applying BASF MasterSeal TC 225HT Gray, containing 4,4'-methylenedicyclohexyl diisocyanate, to concrete surfaces on the top of a parking garage with paint rollers. This condition potentially exposed employees to respiratory hazards associated with 4,4'-methylenedicyclohexyl diisocyanate.  Note:  The requirements applicable to construction work under this section are identical to those set forth at 29 CFR 1926.103.
