OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire hazards. On or about March 21, 2016 employees performing oil well servicing activities worked from the racking board without the use of a emergency escape auxiliary line exposing employees to burn hazards.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by high pressure oil, metal shrapnel, sludge from a blow-out. On or about March 21, 2016, at the Crownquest Hilger High#5, employees while tripping tubing down the well hole were exposed to struck-by hazards when an unexpected pressure release occurred through the top of the blowout preventer.
