29 CFR 1926.55(b): To achieve compliance with paragraph (a) of this section, administrative or engineering controls were not implemented whenever feasible. When such controls are not feasible to achieve full compliance, protective equipment or other protective measures were not used to keep the exposure of employees to air contaminants within the limits prescribed in this section. Any equipment and technical measures used for this purpose must first be approved for each particular use by a competent industrial hygienist or other technically qualified person.  a. Myles Construction Services, LLC worksite located at 8229 Sycamore Woods Ln., Holland, Ohio:  On or about March 9, 2016 the employer did not ensure engineering controls were in place to prevent employees from being overexposed to carbon monoxide.  An employee was overexposed to carbon monoxide at 226 ppm as an 8 hour time weighted average exposure.  This exposure exceeded the OSHA Permissible Exposure Limit (PEL) by 450%.     Step 1: A written detailed plan of abatement shall be submitted to the Area Director outlining a schedule for the implementation of engineering and/or administrative measures to control employee exposures to hazardous substances as referenced in this citation.  This plan shall include, at a minimum, target dates for the following actions which must be consistent with the abatement dates required by this citation:    (1)  Evaluation of engineering /administrative control options;  (2)  Selection of optimum control methods and completion of design; (3)  Procurement, installation and operation of selected control measures;  (4)  Testing and acceptance or modification/redesign .    All proposed control measures shall be approved for each particular use by a competent industrial hygienist or other technically qualified person.  90 day progress reports are required during the abatement period.    Step 2: Abatement shall have been completed by the implementation of feasible engineering and/or administrative controls upon verification of their effectiveness in achieving compliance.    Feasible engineering controls include, but are not limited to equipping gasoline operated concrete trowel with a carbon monoxide scrubbing system and the use of ventilation systems.
