29 CFR 1910.147(a)(3)(i): The employer did not utilize procedures for affixing appropriate lockout devices or tagout devices to energy isolating devices, and to otherwise disable machines or equipment to prevent unexpected energization, start up or release of stored energy in order to prevent injury to employees. At the time of the inspection, the band saw operator was changing the saw blade without affixing a lock to the electric panel disconnect. The employee was turning the switch to off but did not secure it with a lock as required by the written machine specific procedure. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.
