29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) Shop - A 240 volt AC electrical circuit breaker, used to power an Amada shear, was turned off prior to servicing the shear but was not locked out, on or about March 18, 2016. b) Shop - An air valve, used to provide pneumatic power to an Amada shear, was turned off prior to servicing the shear but was not locked out, on or about March 18, 2016. The valve was tagged out but locking hardware was available at the facility to lock it out.
