29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts and crushing hazards:    (a) Building No. 2 - Catbridge Converting Machine 136: The converting machine did not have the unwinding tail feed stock and shaft guarded and other rotating shafts and rollers of the converting process guarded, exposing employees to caught-on, crushing and ingoing nip points hazards.    (b) Building No. 3 - Duplex Converting Machine 72: The converting machine did not have the unwinding tail feed stock and shaft guarded; rotating shafts and rollers guarded; and gears into which the rewinding shaft ends were inserted guarded.  This exposed employees to caught-on, crushing and ingoing nip points hazards.     (c) Building No. 3 - Dusenberry Duplex Converting Machine:  The converting machine did not have the unwinding tail feed stock and shaft guarded and other rotating shafts and rollers of the converting process guarded adequately, exposing employees to caught-on, crushing and ingoing nip points hazards.    (d) Building No. 3 - Jennerjahn Converting Machine: The converting machine did not have the ingoing nip point of the conveyor tail end guarded, exposing the machine operator(s) to caught-in/pinch point hazards.    (e) Building No. 3 - IMC Converting Machine: The converting machine did not have the unwinding tail feed stock and shaft guarded, and other rotating shafts and rollers of the converting process guarded, exposing employees to caught-on, crushing and ingoing nip points hazards.    (f) Building No. 1 - Catbridge Converting Machine 118: The converting machine did not have the unwinding tail feed stock and shaft guarded and other rotating shafts and rollers of the converting process guarded, exposing employees to caught-on, crushing and ingoing nip points hazards.    (g) Building No. 1 - Catbridge Converting Machine 106: The converting machine did not have the unwinding tail feed stock and shaft guarded, and other rotating shafts and rollers of the converting process guarded, exposing employees to caught-on, crushing and ingoing nip points hazards.    (h) Building No. 1 - Catbridge Converting Machine 92: The converting machine did not have the unwinding tail feed stock and shaft guarded and other rotating shafts and rollers of the converting process guarded, exposing employees to caught-on, crushing and ingoing nip points hazards.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:    (a) Employees are exposed to oils, lubricants, solvents, glues and other materials used in manufacturing operations and performance of maintenance work activities. Training was not provided regarding the Hazard Communication 2012 standard and new Global Harmonization (GHS) label elements (product identifier, signal word, pictogram, hazard statement(s), and precautionary requirements) and new safety data sheet (SDS) format.
