29 CFR 1910.269(a)(3)(i)(C): Information about the design and operation of the host employer's installation that the contract employer needs to make the assessments required by this section;   a.  Unit 3, Cooling Tower - The host employer (Georgia Power) did not provide the contract employer (Fluor) electrical circuit drawings or other relevant information needed for determining whether conductors to be removed from Cooling Tower conduit were disconnected and de-energized. The failure to provide adequate circuitry information prevented the transfer of needed information such as the nominal voltage on lines and equipment to be removed during the Fluor demolition process.

29 CFR 1910.269(a)(4): Extent of briefing. Existing characteristics and conditions. Existing characteristics and conditions of electric lines and equipment that are related to the safety of the work to be performed shall be determined before work on or near the lines or equipment is started. Such characteristics and conditions include, but are not limited to:  a.  Unit 3 Cooling Tower - Georgia Power did not provide Fluor management sufficient information for identifying conductor cables that were energized before the demolition project began.  The failure to provide adequate circuitry information prevented the Fluor employee in charge from a briefing transfer of needed information such as the nominal voltage on lines and equipment to be removed during the demolition process.

29 CFR 1910.269(d)(2)(iv)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy;  a.  Plant Bowen, Unit 3 Cooling tower - The employer did not develop and issue a specific clearance (tagout procedure) which included specific procedural steps necessary for working on the de-icing circuits for tower 3.  Georgia Power Company was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.269(d)(2)(iv)(B), which was contained in OSHA Inspection Number 900009, Citation Number 1, Item Number 6b and was affirmed as a final order on December 5, 2014, with respect to a workplace located at Plant Bowen, 317 Covered Bridge Rd SW, Cartersville GA 30120.

29 CFR 1910.269(d)(6)(vii): Before starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergizing of the machine or equipment have been accomplished. If normally energized parts will be exposed to contact by an employee while the machine or equipment is deenergized, a test shall be performed to ensure that these parts are deenergized.  a.  Plant Bowen, Engine Generator House 120/208 Distribution Panel - On or about February 19, 2016 the Georgia Power authorized employee(s) did not verify the absence of voltage prior to work starting on the circuits and equipment.  Georgia Power Company was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.269(d)(6)(vii), which was contained in OSHA Inspection Number 900009, Citation Number 1, Item Number 14b and was affirmed as a final order on December 5, 2014, with respect to a workplace located at Plant Bowen, 317 Covered Bridge Rd SW, Cartersville GA 30120.
