29 CFR 1926.1425(c)(3): The employer did not make sure that the materials being hoisted when employees were hooking, unhooking, or guiding the materials were rigged by a qualified rigger. Construction site, On or about February 19, 2016 employees were exposed to struck-by hazards when the concrete steel form gave way and fell, while an employee was secured to the form.
