29 CFR 1910.146(d)(2): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not identify and evaluate the hazards of permit spaces before employees entered: a) Prior to the entry into the 4-C pulverizer on March 20, 21016 the employer did identify all the hazards in that they did not confirm that the burner line was isolated and cleaned out to prevent re-entry of the residual coal left in the lines back into the discharge chute.
