29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. In the roof truss framing, fall protection was not provided to an employee doing framing work, exposing the employee to a 14 foot 4 inch fall hazard, on or about 3/22/16.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. At the jobsite, the portable extension ladder used to access the roof truss system extended one rung (12 inches) past the landing surface, exposing an employee to a 10 foot fall hazard, on or about 3/22/16.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:  a.  At the jobsite, the portable extension ladder used to access the roof truss system was not secured from displacement, exposing an employee to a 10 foot fall hazard, on or about 3/22/16.
